Is long-term treatment with inhaled steroids in adults hazardous?

چکیده
Since the introduction of inhaled steroids for the treatment of asthma, numerous studies have shown that they have an advantageous therapeutic index in relation to oral steroids, as well as high potency. They certainly represent a major 3:dvance in asthma treatment. Initially, inhaled steroids were used in moderate doses and primarily as a substitute for oral steroids. In recent years we have seen a tendency to prescribe inhaled steroids in higher doses than previously used [1, 2]. They have also been advocated in low doses at an earlier stage in the treatment [2, 3]. If steroids are to be used at an earlier stage in asthmatics with relatively mild symptoms, any significant side-effect must be considered on its own, rather than being compared with the side-effects of oral steroids. The question of how harmless the treatment with topical steroids is must be raised.

Do patients with COPD benefit from treatment with inhaled corticosteroids?
In the past few years, treatment with inhaled corticosteroids has become increasingly important in asthma [1, 2]. It appears that inflammation of the airway wall is a major pathophysiological mechanism underlying asthma [3], and perhaps also chronic obstructive pulmonary disease (COPD) [4]. Some long-term studies in asthmatic patients over 1 and 2 yrs have shown that maintenance treatment with ...

متن کاملAseptic femoral head necrosis in a patient receiving long term courses of inhaled and intranasal corticosteroids.
Aseptic (avascular) necrosis of the femoral head in adults has been associated with a variety of disease entities. It is also recognized as a potential complication of systemic corticosteroid therapy. Inhaled corticosteroids are the first line anti-inflammatory agents for the long term treatment of asthma. However, long term treatment of asthma with inhaled corticosteroids has been accompanied ...
متن کاملLong term clinical comparison of single versus twice daily administration of inhaled budesonide in moderate asthma.
BACKGROUND Inhaled steroids are widely used in the treatment of mild to moderate asthma. However, long term compliance with inhaled steroids is poor and administration of a single daily dose may improve compliance. METHODS A double blind, randomised study was performed to determine whether inhaled steroids given once daily at bedtime are as efficacious as a twice daily regimen in the long ter...
متن کاملInhaled Steroids Modulate Extracellular Matrix Composition in Bronchial Biopsies of COPD Patients: A Randomized, Controlled Trial
RATIONALE Smoking and inflammation contribute to the pathogenesis of chronic obstructive pulmonary disease (COPD), which involves changes in extracellular matrix. This is thought to contribute to airway remodeling and airflow obstruction. We have previously observed that long-term treatment with inhaled corticosteroids can not only reduce bronchial inflammation, but can also attenuate lung func...
